Commited to keeping local youth in the game, Salem Health is proud to offer free sports physicals to support athletes' health and safety. Sports physicals will be available for middle and high school students (6th- 12th grade).

What To Bring:
*Parent/guardian (not required if youth is 15+ years old)
*Completed OSAA form. Copies will also be available at the event.
